Bring your medications with you. Bring your medications with you so the nurse will know what pills you take regularly. Our nurse will confirm your medications with your physician so they can be ordered and delivered from the pharmacy. All medication must be supplied by Buckshon’s Pharmacy. If you have a Power of Attorney and Representation Agreement bring him/her with you. If you have assigned someone with Power of Attorney you will need to provide copies of the Power of Attorney form to both Finance and your nurse. Bring clothes and other personal items. Bring several (5-7) changes of clothes. Bring clothes in a separate container so they can be sent directly to laundry for immediate labeling. This helps us to identify which are your clothes which reduces the potential for loss of articles. We suggest you bring the following: comfortable clothing, a housecoat or bathrobe, current sleeping garments, rubber soled and properly fitting footwear and 7 – 8 undergarments. Before you come, have your hearing aide, glasses, dentures and equipment (walker, wheelchair, cane) professionally labeled. Haro Park Centre cannot be responsible for lost or stolen items. Proper labeling ensures identification of ownership of items. Notify people of your change of address. If you have a subscription to newspapers, magazines, etc. notify them of your change of address and have them redirected to Haro Park Centre. Keep it simple - Moves can be stressful! Arrangements can be made for items such as cable (charges for cable will be added to your monthly care charges) and furniture, television, and /or pictures to arrive at a later date. The day you move in may be a stressful day for you and your family so keeping things simple will help. We will follow up with you in a few days after your admission.
